Fulltext PDFColorectal Cancer (CRC) is the commonest gastrointestinal malignancy and the second commonest cause of cancer death. Emergency presentations constitute about 20% of admissions for CRC. The author’s perspective on the current arguments and evidence that drives the decision-making in emergency colorectal surgery for complications (obstruction, perforation and bleeding) of colorectal cancer is expressed. The global disparities and improvement strategies are discussed. A literature search of the MEDLINE (PubMed) database, Cochrane library, and Science citation index was performed to identify original published studies on CRC emergencies and the management from 1984-2025. The goal of the surgeon is to control in-hospital mortality, local recurrence of CRC and quality of life. There is a paradigm shift in the management of emergency presentations of CRC towards temporization and semi-elective surgery. The timeliness and adequacy of management of the complications of CRC depend on an interaction between patient, disease and surgeon-related factors.
